                         1.  FORM NO.9
                         2.  FORM NO.1 (Medical certificate from MBBS Doctor) with blood group
 

Enclosure :
                         1.  Residence Proof :
                                      a)  Attested copy Voter card OR
                                      b)  Valid passport OR
                                      c)  LIC cover note at least six months old with latest receipt of payment OR
                                      d)  Employer certificate issued by the competent authority with I-Card in case of employee of state

                                           or central govt. OR
                                      e)  If an applicant does not have any of the proof mentioned above he may attach copy of Ration

                                           Card with affidavit.
 
Note :   Following documents accepted as proof of Address supported by Affidavit attested by Public Notary /Magistrate.
          1.  Telephone Bill OR
          2.  Electricity Charges Bill OR
          3.  Water Charges Bill.
          4.  Sr. Citizen Identity Card issued by Social Welfare Department U.T., Chandigarh OR
          5.  Identity Card issued to the Ex-serviceman, by the Zila Sanik Board
